Rhode Island Department of Health Community-Driven Outreach for Hypertension Management and Diabetes Prevention Short-Term Project Opportunity Memo and Application The Rhode Island Department of Health (RIDOH) Diabetes and Cardiovascular Health Program (RIDCH) invites applications from eligible organizations and agencies to apply for a Short-Term Project Funding Opportunity to create and implement products or events that aim to promote hypertension management and diabetes prevention in Providence communities. Products and events should also promote use of available preventive health services and community resources to address social determinants of health (SDOH) needs. The funding period for this Short-Term Project will begin approximately April 1, 2026 through August 31, 2026. A minimum of ten (10) organizations will be awarded up to $4,999 each.
