About Collective Impact Grants

The Blue Cross and Blue Shield of Louisiana Foundation is committed to improving the health and wellbeing of Louisianians. Serious health issues in Louisiana disproportionately impact the economic vitality, educational attainment and quality of life in the state and in communities.

Following the success of our first collective impact grant program – _Challenge for a Healthier Louisiana_ – we know that grassroots coalitions are the key to making long-term changes in the trends of Louisiana’s overall health. That’s why we’re challenging communities across Louisiana to build effective teams to tackle their most pressing health problems.

Collective Impact Grants support community-wide efforts addressing complex issues in Louisiana. Proposals may cover a wide range of issues in health, education or well-being to meet the unique needs of each community. Examples of focus areas could include, but are not limited to: heart disease, diabetes, mental health, obesity, or disparities in access, quality or cost. Through Collective Impact Grants, the Foundation will help Louisiana communities reshape their environments to support healthy lives for all.

About Healthcare Workforce/Collective Impact Grants

Across Louisiana, there are critical shortages in the healthcare workforce, including, but not limited to: general practitioners, dentists, specialists, nurses, mental health providers and other allied health professionals.

These gaps contribute to Louisiana’s health disparities and limit healthcare services in areas of the state where it’s needed most.

The Foundation challenges Louisiana communities to address this major public health concern. We are seeking community-level proposals that build sustainable and lasting solutions for Louisiana’s healthcare workforce shortage.

Eligibility

* All organizations applying for grants or sponsorships must:

* Be designated a 501(c)(3) nonprofit organization by the IRS.

* We only give to nonprofit organizations working for charitable, religious, scientific, literary and other organizations under Internal Revenue Code section 501(c)(3).

* Be registered “in good standing” with the Louisiana Secretary of State.

* All organizations must be up-to-date on their filings with the Louisiana Secretary of State.

* Be domiciled in Louisiana.

* We only make gifts to organizations who are based in Louisiana and working for the welfare of Louisianians

* Matched funds required: a 1:1 match is required. Up to 50% of the match may be given in-kind.

Ineligibility

* The Foundation does not make grants to the following under any circumstances:
* Providers on contract with Blue Cross and Blue Shield of Louisiana
* Programs or events outside of Louisiana
* Organizations that are not tax-exempt
* Political campaigns, candidates, parties or partisan activities
* Sectarian, denominational or religious organizations for support of theological functions
* Athletes or athletic teams
* Beauty pageants
* Direct funding of labor, fraternal or veterans’ organizations
* Students raising funds for travel to an out-of-state competition
* Parent-teacher organizations
* Capital projects or campaigns
* Organizations or causes that do not complement the Foundation’s mission
