    Eligible Organizations

  • Must be a publicly supported charity recognized as tax exempt under section 501(c)(3) of the Internal Revenue Code
  • Must offer opportunity and service to all, regardless of age, race, creed, gender, religion, disability, sexual orientation, and ethnicity
  • Must be dedicated to serving young people aged from infancy through 26 years old
  • Must serve the state of Michigan, with concentration on southeastern Michigan

    Application Timeline

    Two grant cycles per year:
  • Spring Cycle: April 15 application deadline for grants beginning July 1 (Grant Committee meeting in June)
  • Fall Cycle: September 15 application deadline for grants beginning January 1 (Grant Committee meeting in November)

Inquiry Forms must be submitted at least 90 days before grant application deadline.

Grant Application Process

Step 1: Review Your Eligibility

  • Ensure your organization meets all eligibility requirements

  • Review Foundation's focus areas for alignment with your proposed project

  • Contact the Foundation with questions: [email protected]
  • Step 2: Submit an Inquiry Form

  • Complete the downloadable fillable PDF Inquiry Form

  • Include the following information:

  • - The problem you are trying to address
    - How you plan to address it
    - Outcomes you hope to achieve
    - How your project addresses Health Equity, Access, or the Social Determinants of Health
    - Your plans for evaluation and sustainability
    - An estimate of the funding needed
  • Important: If targeting a specific grant deadline, the Inquiry Form must be received at least 90 days before the application deadline

  • Email completed form to: [email protected]

  • Inquiry Forms are accepted at any time
  • Step 3: Feedback from an Impact Director

  • Impact Directors review submitted Inquiry Forms throughout the year

  • Once reviewed, an Impact Director will contact you with next steps

  • If your request aligns with the Foundation's mission and funding priorities, you will be invited to meet with staff

  • Meetings are scheduled at least 60 days prior to upcoming grant deadlines

  • Organizations will be notified via email if invited to submit a full application

  • Email notification will allow at least 45 days to work on the full request
  • Step 4: Submit Full Grant Application (If Invited)

  • Online grant portal opens 30 days prior to each application deadline

  • Submit application package through the online grant portal

  • Application Deadlines:

  • - April 15 for July 1 grant start date
    - September 15 for January 1 grant start date

    Required Application Materials:

  • SMART Objectives Table

  • Planned Participant Table

  • Other required attachments (as specified in application requirements)

  • Budget with notes
  • Step 5: Decision Notification

  • Applications reviewed at Grant Committee meetings (June or November)

  • Funding decision notification via email within 10 business days of committee meeting
