  • Help select students to receive a one-year Ballet Wichita scholarship

  • Wichita State Students: May be eligible to receive college credit for participating in the program

(Yes/No) - If yes: Describe current involvement, where, and in what capacity - Note: Dance teaching experience is NOT required
  • Availability:
  • - Indicate availability for weekdays (Monday-Friday) between September and December 2025 - For each day, select all time slots available: - 9am-11am - 11am-1pm - 1pm-3pm - Leave blank for days when unavailable
  • Essay/Open-Ended Question:
