3. Submit one written letter of reference on letterhead

- Future plans statement: outline degree completion plans, including whether you plan to work while attending school, professional goals, and how the degree helps achieve them
- Brief statement: explain why seeking financial assistance and note any special circumstances
- One official letter of reference on letterhead
- Official school transcripts
- Applicant signature and date

- Deadline: March 31 (postmarked or received)
- Reference Letter: Must be written on letterhead and submitted by mail, in person, or attached to form. Email references NOT accepted.
