Grant Evaluation & Reporting

1. Reporting Timeline: Provide two-week notice when program/event has occurred and allocation check is needed

2. Required Reporting Information:
- Organization Name (required)
- Contact Name (required)
- Date of Program (required)
- Purpose of Grant/Program/Trip/Event (required) - Tell about what was funded
- Number in Attendance/Served (required) - Report how many people participated
- Detailed Information on Expenditures (required) - List budget for the event or the portion the grant funded
- Marketing Materials (required) - Submit examples of marketing/publicity (max file size: 50 MB) that show how the Birmingham Jewish Federation was recognized as a sponsor
- Evaluation Information (required) - Describe how the program was evaluated and report results of that evaluation
- Other Pertinent Information (required) - Include any other valuable information
