Dramatic increase in donors or referrals due to specific efforts of hospital champion, donation committee or team members

Significant process change that improved donation outcomes or family experience

Donation event that greatly increased community awareness of donation

Memorial or event honoring organ donor patients

Collaboration with local media on donation and transplantation stories

Who Can Nominate:

Anyone can submit a hospital for consideration

Timeline:

2025 nominations are CLOSED

Winners announced virtually in January at Georgia Hospital Association's Patient Safety and Quality Summit

Background:
Named after John Ware (kidney transplant recipient) and his wife Barbara (organ donor). The couple dedicated their retirement to encouraging organ donation in their community.

Application Process

1. Submit Nomination Online:
- Nominations submitted using online form
- Form includes hospital identification and specific examples of initiatives/activities

2. Required Information:
- Hospital name and location (within LifeLink of Georgia service area)
- Specific examples of donation support initiatives, events, or activities
- Description of how the hospital demonstrated commitment to organ and tissue donation
- Details of any process improvements, community events, media collaboration, etc.

3. Submission Deadline:
- Currently CLOSED for 2025

4. Selection Process:
- Selection committee reviews all nominations received by deadline
- Two winners chosen annually
- Winners announced in January
