Overview

Program Description The Asthma Home Visit Collaborative (HVC) grant program aligns with the CDC initiative, Controlling Childhood Asthma and Reducing Emergencies (CCARE) and EXHALE Technical Package. Activities are designed to improve childhood asthma outcomes and prevent childhood hospitalizations and emergency department visits. Recipients of the award will provide in-person and/or virtual asthma home visiting services and implement evidence-based EXHALE strategies across multiple sectors: Education on asthma self-management;eXtinguishing smoking and exposure to second-hand smoke;Home visits for trigger reduction and asthma self-management education (AS-ME);Achievement of guidelines-based medical management;Linkages and coordination of care; andEnvironmental policies or best practices to reduce indoor and outdoor asthma triggers. Grantees will implement EXHALE strategies as a coordinated package (as outlined in Performance Measures), taking into consideration the critical social and cultural context of individuals with asthma and their communities. Programs will leverage partners to expand their reach in existing communities and seek expansion into new communities. Grantees should actively seek referral sources and partnerships throughout the grant period. Referral sources may include, but are not limited to, hospitals, health systems, health care providers, schools, community-based organizations, community health workers, and child care centers. Partnerships may also include, but are not limited to, housing, transportation, and home weatherization. As home visiting programs are in a unique position to deliver EXHALE strategies as a package, programs will work in partnership to create the HVC. Each grantee will have its own service area and clients, however, the HVC will serve as a learning community where successes, challenges, opportunities for expansion, policy, reimbursement, and quality improvement initiatives can be discussed. Funding will be for the period of July 1, 2025 – June 30, 2026, through the Tobacco Settlement Recovery Fund and is subject to appropriation. Approximately $116,000.00 will be available for award. Applicants must submit a project plan that covers the 12-month period and how the award will be executed. The project plan should include necessary detail to enable the agency to manage the grant agreement activity against planned project performance. The application will be reviewed, evaluated, and funded based on program need, applicant experience and capability to implement grant activities, and availability of funds.
