Application Process

Submission Deadline: October 15, 2026

Current Status: Applications are not currently being accepted. Check back in Summer 2026.

Required Documentation

All of the following must be included with your application:

1. Completed Agency Application form
2. Current list of Board of Directors with contact information and business/organization affiliation
3. Most recent IRS Form 990 tax return (or most recent year-end financial statement if not applicable)
4. Operating budget for current year or projected budget for next year
5. Copy of most recent Audited Financial Statement (or reviewed financial statement if not audited)
6. Program Results for 2023
7. Program Goals for 2024
8. Federal Employer ID Number (EIN) documentation
9. Supplemental Fundraising request for each 2024 planned fundraiser
10. For first-time applicants: Federal IRS Letter and Tax Exemption Letter

Important Note: Applications missing any required documentation will not be accepted for review.

Submission Format

• Submit one copy of completed application
• Deadline: October 15, 2026
• Method: Via Franklin County Area United Way (specific submission method not detailed on provided content)

Key Definitions

Number Served: Total number of people served, counted only once regardless of multiple visits

Units of Service: Total count of each individual service provided (accounts for multiple services per person)

Administrative Cost: Use the Administrative Cost % Calculation Worksheet (available on website) with your most recent Form 990
