The United States is aging. The number of older adults, individuals ages 65 years and older, is growing rapidly. Health systems are working to find ways to meet the complex needs of the growing population of older adults. To address these challenges, in 2017, The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, set a bold vision to build a social movement so that all care with older adults is age-friendly care, which:
* Follows an essential set of evidence-based practices;
* Reduce or prevent harms; and
* Aligns with What Matters to the older adult and their family caregivers.
Age-Friendly Health Systems
Becoming an Age-Friendly Health System entails reliably providing a set of four evidence-based elements of high-quality care, known as the “4Ms,” to all older adults in your system.
* _What Matters:_ Know and align care with each older adult's specific health outcome goals and care preferences including, but not limited to, end-of-life care, and across settings of care.
* _Medications:_ If medications are necessary, use age-friendly medications that do not interfere with What Matters, Mentation or Mobility.
* _Mentation:_ Prevent, identify, treat and manage depression, dementia and delirium across settings of care.
* _Mobility:_ Ensure that older adults move safely every day in order to maintain function and do What Matters.
When implemented together, the 4Ms represent a broad shift by health systems to focus on the needs of older adults.
To continue to enhance this work, The John A. Hartford Foundation is providing scholarships for a cohort of Age-Friendly Fellows who will participate in the Next Generation Leaders Fellowship. These applicants will be asked to implement the 4Ms Framework at their organization for their change project in the fellowship program. This scholarship covers fees for the program year: $13,995. However, it does not cover travel expenses and any other additional expenses incurred during the program.
Next Generation Leaders Fellowship
America’s hospitals and health systems are transforming to meet the changing needs of our patients and our communities. It is vital that future health care leaders have the knowledge and skills to adapt while navigating these changes.
The AHA Next Generation Leaders Fellowship focuses on developing leaders and empowering them to bring about real and lasting change in the hospital and health systems in which they serve. This annual fellowship is a unique opportunity to be paired with an executive-level mentor who will guide you through the planning and execution of a year-long capstone project at your hospital or health system, tackling key issues and challenges affecting health care affordability, cost, quality and safety.

* The John A. Hartford Foundation is providing scholarships for a cohort of Age-Friendly Fellows who will participate in the Next Generation Leaders Fellowship.
* Applicants that will be considered are ones who implement one or more of the 4Ms Framework at their organization as part of their capstone project in the fellowship program.
* This is a competitive fellowship program for health care leaders who are accomplished in their careers and expected to rise to the top of their fields in the next five to ten years.
* They are leading (or on the verge of leading) transformation efforts at department, organization, enterprise and/or system levels.
* The program aims to select a cohort of leaders who bring diverse perspectives and experiences from a variety of settings: clinical, strategy and operational backgrounds with a strong foundation in quality, safety, affordability and are looking to expand skills to effect change at their organization.
* Up to 42 fellows, including 12 Age-Friendly Fellows, will be accepted into each fellowship class.
_You must be a member of this organization to apply for this award - become a member. Although applicants will be accepted from non AHA-member organizations, preference will be given to AHA member organization-employed applicants._
