Required Materials:
1. Application form, including personal statement on how you exemplify the values of Theta Phi Alpha (2,500 characters or fewer)
2. College transcripts
3. FAFSA form
4. Expected Family Contribution (EFC) form
5. Recommendation letter completed by an advisor, faculty member, or manager (you will need to provide their email address)
6. For undergraduates: Chapter Advisor's name and email address
