Overview
SCAN Health Plan's Community Giving Program offers grants of up to $15,000 to support established older adult nutrition programs providing congregate meals, home-delivered meals, food boxes, older adult food pantry services, Commodity Supplemental Food Programs, or senior produce distribution in SCAN's newly added counties. Award notifications November 12, 2026; projects start December 1, 2026.

Community-serving nonprofits and government agencies providing nutrition services to adults ages 55 and older in Sonoma County, CA or Chambers or Liberty County, TX. Applicants select one eligible county for the project.
Ineligibility
Current SCAN Community Giving grantees, and organizations whose most recent SCAN grant ended after December 1, 2025. Only one application per organization across all SCAN Fall/Winter 2026 funding opportunities.
