THE ENTITY PROVIDES COMMUNITY HEALTH WORKER PROGRAMS, PROGRAMS TO IMPROVE COMMUNITY ACCESS TO QUALITY FOOD, PROGRAMS TO CONNECT PEOPLE WITH HEALTH AND HUMAN SERVICES, AND PROGRAMS TO INCREASE THE CAPACITY OF INDIVIDUALS AND COMMUNITY GROUPS TO EFFECTIVELY INFLUENCE SOCIAL DETERMINANTS OF HEALTH.
