The Foundation for Healthy Communities (FHC) is a 501(c)(3) nonprofit organization based in Concord, New Hampshire. Originally founded in 1968 as an education and research organization for the New Hampshire Hospital Association (NHHA), the Foundation was reorganized in 1995 to adapt to the changing healthcare system.
