Application Process

Required Information:

2. Employment Verification
- Confirmation of minimum 12 months consecutive YMCA service
- Current employment status
- Employment standing verification

3. Financial Information
- Confirmation that all other financial resources exhausted
- Details of exhausted resources (family, retirement fund, banks)
- Total monthly household income
- Household member count
- Current government/nonprofit assistance received
- Type of assistance currently receiving (WIC, food stamps, rent assistance, etc.)
- History of government or charitable reliance

4. Assistance Request Details
- Type of assistance requested
- Amount of assistance requested
- Detailed explanation of circumstances requiring assistance
- Timeline of qualifying event (within last 180 days)
- Financial alternatives already explored (minimum of three)
- Previous award history from this program

5. Support and Stability Factors
- Actions willing to take for financial stability
- Financial planning or credit counseling participation
- Criminal history disclosure
- Nature of crimes and rehabilitation (if applicable)

6. Consent and Authorization
- Story sharing options (anonymous sharing, thank you note, or privacy preference)
- Agreement to terms and conditions
- Authorization for background check and reference checks
- Privacy policy consent
