Overview

The King County Department of Community and Human Services (DCHS) is pleased to release this Request for Proposals (RFP) for the Vital Program Supportive Housing (Project) initiative with funding from Housing and Supportive Services. Vital is the name adopted by the Familiar Faces Intensive Care Management Team, which provides comprehensive and integrated services to eligible adults who need an intensive level of community-based support and may be experiencing homelessness. This RFP makes available $428,772 over the course of four years with a first-year annual amount of $99,505 to operate the supportive housing component of the Vital program. Funding for the Project will be used to provide Permanent Supportive Housing with onsite Housing Support Services and coordinated Case Management with the Vital intensive care management team for 20 eligible individuals. Eligible individuals are those who have been identified by the Vital team who have mental health and/or substance use conditions and have been booked into the King County Jail four or more times in a 12-month period. The selected housing provider shall coordinate with the Vital team to ensure that individuals identified as eligible for this program obtain and maintain housing stability. The Vital team consists of the Vital Steering Committee and Vital-Familiar Faces Strategy evaluation staff, who will identify and engage with eligible individuals. Background The Health and Human Services Transformation Familiar Faces Strategy, known as the Vital Program, is a King County initiative that originates from the Behavioral Health and Recovery Division (BHRD) within the King County Department of Community and Human Services (DCHS), which is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) qualifying as a health care plan and health care provider for mental health services and as a health care provider for chemical dependency services. The Vital program provides comprehensive and integrated services to adults who are experiencing behavioral health challenges (mental health conditions and/or co-occurring substance use issues), need an intensive level of community-based support, may be experiencing homelessness and are involved in the criminal justice system. Objective The objective of this Project is to provide Permanent Supportive Housing with onsite Housing Support Services and coordinated Case Management, with the Vital team, for 20 eligible participants who have mental health and/or substance use conditions and have been booked into the King County Jail four or more times in a 12-month period.
