Overview

Upper Valley Community Health Equity Partnership Grant Program

Status: First round of projects selected in October 2022 for projects ending May 31, 2023.

Geographic Scope: White River Junction District of Vermont Department of Health

Focus Areas: Reducing health disparities for:

  • Black, Indigenous, and People of Color (BIPOC) communities

  • Lesbian, Gay, Bisexual, Transgender, Queer, Intersex, Asexual and plus (LGBTQIA+) communities

  • Projects addressing root causes of mental health challenges, substance use disorder, isolation, chronic health conditions, and safety concerns
  • Funding Pool: Total of $150,000 available

    Eligibility:

  • Individuals

  • Grassroots Organizations (fiscal sponsor support available)

  • Nonprofit Organizations

  • Businesses

  • Federally and non-federally recognized Native American Indian tribes and Alaska Native entities

  • Governmental Entities

  • Organizations not located in VT eligible if they serve people in White River Junction District and demonstrate ability to address the Problem Statement
  • Key Features:

  • Designed to reduce barriers for informal, small, and new entities

  • Multiple application formats accepted

  • Short, simple application (estimated 2-4 hours to complete)

  • Up to 10% of total award provided at beginning of grant period

  • PHC works with grantees on case-by-case basis for cash flow concerns

  • Clear and convincing plan to address health disparities outlined in Problem Statement
