- Average GPA
- Extracurricular involvement (clubs, sports, music, etc.)
- School/extracurricular awards and accomplishments
- Post-secondary plans
- Anticipated scholarship use

3. Outdoor Experience Section - Narrative responses required:
- Hunting experience and accomplishments
- Fishing experience and accomplishments
- Shooting sports involvement and accomplishments
- SDYHA involvement and accomplishments
- Conservation/habitat volunteerism and projects
- Agricultural involvement and experience

4. Certification & Consent
- Certify all information is accurate and complete
- Acknowledge that false information may result in disqualification
- Consent to use of name and associated content in promotional materials (unless otherwise specified)
- Signature and date required
