The Transformation Grants for IDD Housing program reflects our commitment to investing in person-focused housing solutions for individuals who experience IDD. The program also provides funding for the exploration of new, inclusive, and community-based approaches that are affordable, scalable, and sustainable. Grants are limited to organizations based in—and directly serving residents of—these two states.Organizations must be a registered 501(c)(3) nonprofit. Organizations using a fiscal sponsor must have an agreement in place at least six months prior to the grant application date, and the fiscal sponsor must demonstrate a five-year history of successful operation.Ineligibility
