Title III-D Health Promotion and Disease Prevention Grant

Program Type: Federal Older Americans Act funds

Geographic Scope: Eleven counties of Southeastern Minnesota (Dodge, Fillmore, Freeborn, Goodhue, Houston, Mower, Olmsted, Rice, Steele, Wabasha, Winona)

Eligibility:

• Non-profit agencies
• For-profit organizations (as approved by the State of Minnesota)
• Grants are NOT available to individuals

Purpose: Funds are intended for evidence-based health promotion programs to serve people 60 years of age and older.

Priority Programs:

• Arthritis Foundation Exercise
• Tai Ji Quan
• Stepping On
• Matter of Balance
• PEARLS (Program to Encourage Active Rewarding Lives)

2. Submit full grant application by deadline (August 20, 2021 for 2022 funding)
3. Required Application Components:
   • Full grant application (form provided by SEMAAA)
   • Budget detailing expenses
   • Explanation of need for service in applicant's service area (outline cities/counties to be served)
   • Statement of anticipated number of units and individuals to be served
   • Demonstration of ability to sustain project beyond grant period
   • W-9 form
   • Certificate of Insurance
   • Minimum request: $10,000 per project
