3. Required Application Components:
- Full grant application (form provided by SEMAAA)
- Budget detailing expenses
- Explanation of need for service in applicant's service area (outline cities/counties to be served)
- Statement of anticipated number of units and individuals to be served
- Demonstration of ability to sustain project beyond grant period
- W-9 form
- Certificate of Insurance
- Minimum request: $10,000 per project
