  • Grants are NOT available to individuals
  • Purpose: Funds are intended for evidence-based health promotion programs to serve people 60 years of age and older.

    - Demonstration of ability to sustain project beyond grant period
    - W-9 form
    - Certificate of Insurance
    - Minimum request: $10,000 per project
