The Thriving Community Impact Fund is more than financial support—it is a catalyst for lasting change, rooted in collaboration and shared purpose. Community Health Impact Programs & Activities We welcome grant applications for programs and activities that enhance the health and well-being of our community members, closely aligned with the Health System’s Community Health Improvement Plan and Community Health Score.
