Application Procedures for Fall 2026 Scholarship

Required Materials:

1. Personal Essay (minimum 1,000 words, typed in 12pt font)
- Describe your background and long-term goals
- Explain why higher education is key to achieving your goals
- Include how extracurricular activities and community service support your education

2. List of Honors/Activities (one page maximum)
- One-page list of honors, activities, and community service
- Resume is NOT accepted

3. Two Letters of Recommendation
- Must be signed and dated within the last six months
- Older letters will not be accepted
- Use letterhead if possible
- Letters from family members will not be accepted

4. Proof of Enrollment
- Proof of acceptance or enrollment for Fall 2026 semester showing full-time status
- Acceptable documents: acceptance letter or course enrollment printout indicating full-time status

5. Official Transcript
- Order through your school's website or via Parchment (if your school uses it)
- Must be received via secured email to info@theresamike.org OR by mail (sealed) from your registrar's office
- Must be received by June 30, 2026 (not postmarked or ordered by that date)

6. FAFSA Report
- Copy of 2026-27 FAFSA/SAR summary, including all pages
- Black out all Social Security numbers

7. AGI Documentation (if FAFSA/CA Dream Act ineligible)
- Page one of your tax return, or your parents' if you are a dependent
- Black out all Social Security numbers

8. Professional Photograph
- 4"x6" professional photograph of yourself

9. Acknowledgement Form
- Complete all sections
- Sign where required
- Incomplete or illegible forms may be disqualified
