Overview

The CAPABLE Model: An Aging in Place Intervention for Older Adults in Central Texas

Geographic Scope: Central Texas (Bastrop, Caldwell, Hays, Travis, and Williamson Counties)

Focus Area: Aging in place for vulnerable seniors

Program Description: Funding to replicate the Community Aging in Place—Advancing Better Living for Elders (CAPABLE) model, developed by the Johns Hopkins School of Nursing. CAPABLE uses a multidisciplinary team approach to help vulnerable seniors live safely in their homes.

CAPABLE Team Composition:

Occupational Therapist (OT)

Registered Nurse (RN)

Handyperson

Service Approach:

Client-directed approach where seniors determine their own goals for functional improvement

Seniors define meaningful daily activities they want to achieve

CAPABLE team honors client decisions

Target Population:

Income Level: Low-income (generally defined as under 250% Federal Poverty Level)

Age: Generally 60 years or older

Location: Central Texas (Bastrop, Caldwell, Hays, Travis, Williamson Counties)

Eligibility Criteria: Low-income older adults who have the desire and cognitive ability to remain in their home but have difficulty with activities of daily living that threaten their ability to remain safely at home

Eligibility: Tax-exempt 501(c)(3) non-profit organizations and/or public entities

Foundation's Commitment: This RFP expands and complements the Foundation's existing commitment to support aging in place for vulnerable seniors.
