Specific Population Community Grant

The Specific Population Community Grant, offered by the New Jersey Department of Health, aims to reduce health inequities among six targeted populations. Four grantees will be selected to provide tailored health navigation services, conduct outreach, and manage program activities. This initiative is designed to foster collaboration between key staff and community members, ensuring comprehensive support and addressing barriers to access, ultimately improving health outcomes and quality of life for New Jersey residents.

The goal of this grant is to support efforts aimed at reducing health inequities among the six identified populations. Four (4) grantees will be awarded, each one focusing on one of six specific populations and being responsible for providing tailored health navigation services, conducting outreach and education, and effectively managing program activities. Successful implementation of the program will rely on a dedicated team, with key staff members working closely with the community to ensure comprehensive support, identify needs, and address barriers to access.
