Smoky Valley Cares Fund - Beyond the Diagnosis

Provides financial assistance for individuals with cancer who may be in financial need and are actively undergoing cancer treatment.

Background: Effective with the 4th Annual Battle of the Buses on September 13, 2015, the fund expanded to offer support beyond screenings.

Geographic Scope: Smoky Valley service area (priority given to residents)

Covered Services:

• Nutrition
• Transportation
• Shelter
• Medicine

Award Details:

• Up to $3,000 in total grants designated annually
• Maximum of $500 per recipient per 12-month period
• Funding is at the discretion of the Fund administrators
• Issued in the form of a check

Eligibility Requirements:

• Must be actively undergoing cancer treatment
• Priority given to patients who reside in the Smoky Valley service area and seek regular medical care with FHCC providers
• Limited funds available

How to Apply

Each application must include:

1. A brief statement of financial need
2. A signed letter from a medical provider of the Lindsborg Community Hospital or treating oncologist
3. Completed application form
