Graduating high school seniors

Minimum GPA of 3.0 on a 4.0 scale

Minimum 40 hours of community service

Participation in South Bay F.O.R. organization and/or local community service

Comprehensive application

High school transcript (grades 9-12, 1st semester)

Permission to Use Image Form (signed by parent(s) if under 18 years of age)

Interview participation with the F.O.R. Scholarship Committee
