Promoting Integration of Primary and Behavioral Health Care: States Grant FORECAST

The Promoting Integration of Primary and Behavioral Health Care program aims to enhance collaboration between physical and behavioral health services. It focuses on improving integrated care models to enhance overall wellness, mental health treatment, and recovery for individuals with mental health and substance use disorders, as well as co-occurring physical health conditions. By promoting evidence-based practices, this initiative seeks to provide comprehensive support for patients, addressing both their physical and behavioral health needs.

The purpose of this program is to: promote full integration and collaboration in clinical practices between physical and behavioral health care; support the improvement of integrated care models for physical and behavioral health care to improve overall wellness and physical health status; and promote the implementation and improvement of bidirectional integrated care services, including evidence-based or evidence-informed screening, assessment, diagnosis, prevention, treatment, and recovery services for mental and substance use disorders, and co-occurring physical health conditions and chronic diseases.
