Program Description

Plan For Life provides a limited number of one-time only legal grants of up to $1,500 toward the cost of legal services that are related to ALS.

Eligible Legal Services

Grants may be used for:
• Durable Powers of Attorney for Health Care or Finances
• Living Wills or Medical Advance Directives
• Estate Planning
• Medicaid eligibility determination and planning
• Other ALS-related legal services

Program Goals

• Provide financial assistance for ALS-related legal planning
• Alleviate stress of living with ALS through adequate legal planning

Application Periods

Four rounds per year:
• Round 1: January 15 – February 15 | Selection: March 1
• Round 2: April 15 – May 15 | Selection: June 1
• Round 3: July 15 – August 15 | Selection: September 1
• Round 4: November 15 – December 15 | Selection: January 1

Selection Process

ALS Ride For Life will select a limited number of applications through a random drawing.

Grant Payment

• Grants will be paid directly to a provider of legal services, not to the patient
• Reimbursement will NOT be made for legal services rendered prior to the grant award date
• Only receives one-time grant only

Program Funding

Funding for this program comes from the efforts of fellow patients who participate in the annual ALS Ride For Life each May. Thanks to the support of the wider ALS community, this assistance is made possible.

Eligibility Notes

• Unsuccessful applicants are encouraged to reapply each quarter
• Program available to persons with confirmed ALS diagnosis
