How to Apply

1. Download the Request for Proposal (RFP) form - Located on the PACT grants webpage for detailed instructions

2. Complete the Application - Found in the Grant Packet

3. Submit via Portal - Applications must be submitted through the PACT Grants web portal
- Important: Proposals submitted through any other method will not be reviewed

4. Confirmation - You will receive an email confirming receipt of your completed application

Application Requirements

  • Complete Information Required: All requested information must be included
  • Attachments: All required attachments must be submitted
  • Multiple Programs: If your organization has multiple programs to request funding for, submit a separate application for each program
  • Incomplete Applications: Applications lacking requested information or required attachments will be considered ineligible

Evaluation

  • A scoring rubric is used to evaluate applications
  • Review the RFP guide to understand what types of programs will be funded
  • All applicants will be notified of outcomes via email
