- Primary: Housing expenses including rent or mortgage payments
- Alternative: If the individual does not have a housing payment, Nurses House will cover medical expenses as outlined in the Service Program Guidelines

How to Apply
Step #1: Medical Documentation
1. Print the Nurses House Health Status Report form
2. Have it completed and signed by your medical provider (physician or nurse practitioner)
3. Proceed to Step #2 once form is signed
Step #2: Gather Required Documents
Upload the following documents:
1. Completed Health Status Report Form (from Step #1)
2. Current or most recent pay stub
3. Pay stubs for anyone else employed in your household
4. Other income statements if applicable:
- SSI (Supplemental Security Income)
- SSD (Social Security Disability)
- Short-term or long-term disability
- Workers compensation
- Food stamps
- Any other income for everyone living in your household
5. Copy of mortgage statement or lease agreement
- If none available: property tax bill or medical bills may be considered
