Mona Flax Breast Cancer Endowment Fund (MFBCEF)

An endowed fund managed by Madison Health Foundation that provides financial assistance for breast cancer screening, diagnosis, and treatment.

Geographic Scope

• Madison County, Ohio and surrounding areas
• Serves patients using Madison Health services

Focus Areas

• Screening mammograms: No out-of-pocket cost for all patients regardless of financial need
• Diagnostic testing: Financial assistance for additional breast cancer diagnostic tests
• Treatment: Financial assistance for breast cancer treatment

Eligibility

• All patients regardless of income or need (for screening mammograms)
• Qualifying patients for diagnostic and treatment assistance
• Patients receiving care at Madison Health

How It Works

1. Patients are first directed to an outside agency: the Breast Cancer Cervical Cancer Program
2. If that organization does not cover mammogram costs, MFBCEF provides assistance

If a patient receives a bill for a screening mammogram, they should contact Patient Financial Services at 740-845-7033.

How to Apply

1. For Screening Mammograms:
   • Call 740-845-7100 to schedule a mammogram
   • You will be directed to the Breast Cancer Cervical Cancer Program first
   • If that program does not cover costs, MFBCEF will provide assistance
   • No separate application required for screening
2. For Diagnostic Testing or Treatment Assistance:
   • Contact Madison Health's Patient Financial Services Coordinators
   • They will work with you to determine eligibility
   • Contact Patient Financial Services at 740-845-7033 or Madison Health Foundation at 740-845-7050
3. If You Receive a Bill:
   • Contact Patient Financial Services at 740-845-7033 immediately
