1. Patients are first directed to an outside agency: the Breast Cancer Cervical Cancer Program 2. If that organization does not cover mammogram costs, MFBCEF provides assistance- Madison Health Foundation: 740-845-7050

- To Schedule Mammogram: 740-845-7100

How to Apply
1. For Screening Mammograms:
- Call 740-845-7100 to schedule a mammogram
- You will be directed to the Breast Cancer Cervical Cancer Program first
- If that program does not cover costs, MFBCEF will provide assistance
- No separate application required for screening
2. For Diagnostic Testing or Treatment Assistance:
- Contact Madison Health's Patient Financial Services Coordinators
- They will work with you to determine eligibility
- Contact Patient Financial Services at 740-845-7033 or Madison Health Foundation at 740-845-7050
3. If You Receive a Bill:
- Contact Patient Financial Services at 740-845-7033 immediately
