Mission Alignment: Programs must be aligned with YouthWell NH's mission to positively impact and improve the mental health and well-being of young people in New Hampshire.

Focus Areas: Youth mental health and well-being; education, resources, tools and supports for children, youth, families, schools and youth-serving organizations

Application Process

1. Submit Application - Complete the YouthWell NH funding application form
2. Application Review - Application form reviewed by YouthWell New Hampshire
3. Meeting - Meeting scheduled to discuss proposal and answer questions (if needed)
4. Award & Funding - If awarded, funds will be released immediately
5. Follow-up & Reporting - Complete questionnaire or schedule meeting to discuss program outcomes

• Amount of funds requested with itemized budget

• Description of need for proposed program/project/initiative (include data and/or anecdotal narrative)
• Description of program/project/initiative (include goals and objectives, target group, who will deliver program, timeline)
• Explanation of how program aligns with YouthWell NH's mission
• Measurable outcomes of the program
• Information on other funding sources (if applicable)
• Plan for program continuation after funding is utilized
• Any additional information to support the application
• Optional: Supporting documentation
