NOTE: The Health Fund strongly encourages concept paper submissions of up to two pages for review and feedback prior to submission. While not required, previous applicants have stated this process was very helpful in development of a full proposal. Concept papers are due by the Pre-Proposal deadline above. Program Overview As we age, we all deserve access to the opportunities we need to maintain our health, well-being, and independence, even as we run into challenges that can arise along the way. That’s why the Health Fund invests in programs, partnerships, and models that equip organizations, health care providers, and policy makers to effectively meet the needs we face as we age. The Healthy Aging Initiative aims to improve the health and wellness of Michigan’s older adults and their caregivers, while reducing the cost of care. To achieve this aim, the initiative seeks to support projects that will collectively make meaningful progress toward improving the quality of care for older adults in the health care system, coordinating care in the community that will improve health outcomes and lower overall costs, and providing more support at home for formal and informal (family) caregivers. Priority Areas The Healthy Aging Initiative has identified priority areas for this year to complement the Health Fund’s Strategic Plan. These are presented for your consideration as you develop your proposal, but your proposal is not required to address a specific priority area. Alzheimer’s Disease and Related Dementias Projects that improve the quality of life for those living with dementia and those who care for them. Elder Justice Develop collaborative approaches to identifying, supporting, and improving the health and quality of life for older adults who are victims of abuse, neglect, exploitation, or discrimination. Caregiving Improve the health or quality of life of family caregivers of older adults with a focus on innovative respite care models and increase retention rates of professional caregivers supporting older adults. Access to Care Increase utilization of primary, behavioral, palliative care or other services for older adults, with a focus on populations that face disproportionate barriers to care.Eligibility
