Middle Park Health Employee Scholarship (1-3 Years Service)

Application Process

1. Verify all CME funds have been utilized through MPH before applying
2. Decide which scholarship best fits you and confirm eligibility
3. Apply online or by mail (printed versions available upon request)
4. Submit completed application with all required materials
5. Application review by scholarship committee
6. Notification of approval status (generally within one month of receipt)

Required Materials

• Completed online or printed application form
• Two letters of recommendation (excluding family members)
- Letters should come from faculty, employers, or other community members familiar with applicant's personal qualities and work habits
• Most recent performance evaluation received from MPH
• Evidence of minimum 1 year service at MPH
• Current employment verification (minimum 30 hours/week)

Important Notes

• All applications must be fully complete to be considered
• Applicants must be studying in the field of healthcare sciences
• Direct payment to institution; no past tuition reimbursement
• Must NOT be under a Performance Improvement Plan (PIP)
• Physicians employed by MPH are NOT eligible
