Middle Park Health Employee Scholarship (1-3 Years Service)

Geographic Scope: Colorado (for Middle Park Health employees)

Focus Area: Healthcare sciences education

Eligibility:

• Currently employed at Middle Park Health (MPH)
• Minimum of 1 year of service at MPH
• Work minimum of 30 hours per week
• Must have capacity to continue work at MPH
• Must be studying in the field of healthcare sciences
• NOT employed as a physician at MPH
• Must have completed all available Continuing Medical Education (CME) funds before applying
• Should not be under a Performance Improvement Plan (PIP) at time of application

Service Commitment: 1 year of service commitment required, beginning from date MPMF disburses payment to educational institution

Consequences of Non-Compliance: If service commitment is not met, employee must reimburse MPH for prorated share of funds before last day of employment

Payment: The foundation pays the institution directly; no reimbursement for past tuition

Review Timeline: Applicants are generally notified of approval status within one month of application receipt

• Most recent performance evaluation received from MPH
