Required Materials:
1. Full Name
2. Chapter affiliation (from comprehensive list of Delta Gamma chapters)
3. Response to narrative questions (see below)
4. Headshot photo (recent picture, max 100 MB, named with first and last name, e.g., AnnaBoydEllington.jpg)

2. What does being a Delta Gamma mean to you?
3. How will receiving a Delta Gamma Foundation merit-based scholarship impact you and your time in college? Include any information about your academic studies, current circumstances, etc. that may be relevant.
4. Why is it important for our Delta Gamma sisters to ensure that programs like merit-based scholarships continues to exist?
5. Is there anything else you would like to share about your experience with the merit-based scholarships or the Delta Gamma Foundation? Anything that could be improved, any positive/negative experiences, any thoughts, etc.
