• Focus on vulnerable and high-risk populations: Prioritize services for populations at high-risk for poor health outcomes
• Reduce barriers to care: Address barriers including geographic location, transportation, language discrimination, health literacy, and financial/social factors
• Social determinants of health: Promote use of funds to improve direct access to healthcare and activities addressing social determinants of health and well-being

Eligibility Requirements

Applicants MUST answer YES to ALL of the following:

Are you a charity or non-profit 501(c)(3) organization?

Are you based in Montgomery, Toombs, or Wheeler County?

Do you offer health services or health education?

Additional Requirements:

Organizations must have existed for at least 12 months

Current or former grantees must have approved 6-month or 12-month program reports

Ineligible Uses of Funds

• Direct Foundation payment for Meadows HCA services
• Campaigns to elect candidates to public office
• Organization endowment campaigns or debt reduction
• Building funds
• Fundraising events
• Start-up costs for new organizations or organizations that have existed for less than 12 months

Background

Meadows New Foundation was established to receive proceeds from the sale of Meadows Health Alliance assets in April 2021. The Foundation is governed by an independent, volunteer Board of Directors comprised of Georgia residents and local leaders. The Board is self-nominating and term-limited. The Foundation gathers community input through Listening Engagements with nonprofit and community leaders to identify local needs and shape grantmaking priorities. Grant cycles align with investment returns to ensure long-term sustainability.
