NOTE: The first step of the process is to submit a request form. Second, if the request has been approved, you will receive an email notification with a request to sign and return a Letter of Agreement (LOA) and provide your electronic banking information (via a secure server). Once the LOA and banking information are provided, we will initiate payment and you will receive an email notification when payment has been completed. Lundbeck US Charitable Fund The Lundbeck US Charitable Fund was established to support organizations who are dedicated to brain health, whether their missions are supporting individuals and families affected by these diseases, building community support systems to improve disease education and care, or advancing innovation and new models to support a brighter future for everyone affected. The challenge of brain health Three billion people are impacted by brain health conditions - that's nearly half the world's population. And people living with brain diseases lose 10-20 years in average lifespan versus healthy individuals. Unfortunately, addressing brain diseases, whether mental health or neuroscience related, is no small feat. Yet the millions of people suffering from these conditions every year are urgently awaiting new ideas, new discovery, and new solutions that can relieve the burden and allow them to be their best. The Lundbeck US Charitable Fund shares Lundbeck's dedication to delivering transformative progress in brain health. But, as an independently managed nonprofit, the Fund plays a unique role in driving meaningful change and fueling progress beyond our walls. We do this through responsible and appropriate charitable donations to organizations and programs that share our dedication to advancing brain health and transforming lives. The Fund does this by distributing high impact grants across our three areas of strategy. Accelerate ongoing community efforts: The Lundbeck US Charitable Fund is a strong and active advocate for organizations in Chicagoland, a large and diverse community with significant areas of need. We thoughtfully partner with organizations that are changing lives every day through localized outreach, interventions and support for some of our most at-risk communities, particularly in the areas of mental health care.Address gaps in care today through community collaboration: We actively seek opportunities to support communities that are at high risk of brain disorders, with a goal to make real progress in supporting vulnerable populations or those whose support networks fall behind other communities in resourcing or reach. One of our areas of focus is the military and veteran community – a population that suffers from a disproportionate burden of mental health disorders. We are amplifying and expanding the reach of incredible organizations in the U.S. who are delivering life-changing impact for our veterans and military families.Fuel truly novel strategies for future progress: We focus on fueling state-of-the-art strategies through innovation acceleration across the community, focusing on those with the potential to redefine the next generation of care for a wide range of brain disorders. Our impact In 2024, the Fund equipped 14 organizations and delivered $4.6 million to advance critical missions in brain health. The Lundbeck US Charitable Fund is dedicated to supporting organizations who share our desire to reduce the burden of brain disorders. The organizations we supported over the last year are making a tangible impact on lives every day, and we are inspired by their persistence and ingenuity to address the burden of brain diseases both locally and globally. Sponsorships At Lundbeck, we are dedicated to advancing brain health and transforming lives. We understand that our corporate social responsibility extends beyond the medicines we make, and we are committed to giving back to the community in a responsible and appropriate way. Sponsorships are funding opportunities offered by a 3rd party (other than education grants, fellowships, or other medical funding requests) that also present a tangible benefit to Lundbeck. Examples of sponsorships are: Local awareness walksCommunity education eventsNational meeting or conference sponsorship Tangible benefits further Lundbeck’s business or policy goals and include, but are not limited to: Journal/newsletter advertisements Exhibit space Banner advertisements Tickets or registrations to a gala, etc. Branding on items associated with the event Focus areas Lundbeck considers sponsorship and charitable contribution requests to organizations whose missions align with Lundbeck’s dedication to advancing brain health and our efforts to build a healthy, sustainable society: Neurological and psychiatric disorders specific to disease states Lundbeck serves Rare brain diseases for which there are few, if any, management options Organizations whose mission is in line with Lundbeck’s purpose and beliefsEligibility

Funding restrictions:In general, Lundbeck does not provide funding for the following requests through its Charitable Contributions program: Request where Lundbeck is asked to be sole supporter or provide more than 1/3 of overall program budget Request where Lundbeck would provide greater than or equal to 25% of the requester’s overall budget in a 12-month period The requester would use funds for a Health Care Provider (HCP), HCP practice or Health Care Organization (HCO); an entity owned/controlled by these individuals; an agent or vendor acting on behalf of these individuals or a non-profit charity in support of these individuals Request is associated with a disease that is not consistent with an on-label disease of an FDA-approved Lundbeck product Request is not compliant with applicable federal and state laws, the PhRMA code and Lundbeck’s policies Requests from organizations that discriminate on the basis of age, political affiliation, race, national origin, ethnicity, gender, disability, sexual orientation or religious beliefsThe donation is not, directly or indirectly, for: the benefit any specific HCP practice, HCO, or other customer.the personal benefit or use of any individual, including any Lundbeck customer.advertising, marketing, or promotion of a specific Lundbeck customer.the performance of any services or in lieu of payment for a fee-for-service activity.development of clinical practice or treatment guidelines or education.capital campaigns, infrastructure, or overhead (e.g., purchase, construction, expansion, or modification of facilities, equipment, or salaries).Focus Areas & Funding Uses
