Kyle Broussard Memorial Fund Grant

Purpose: Assists families from Southwest Louisiana with children receiving treatment for cancer.

Background: The Kyle Broussard Memorial Fund was established as a living legacy for Kyle, who died of cancer in 2007 at the age of 21.

Geographic Scope: Allen, Beauregard, Calcasieu, Cameron, or Jeff Davis Parish

Eligibility Requirements

• Applicants must be residents of Allen, Beauregard, Calcasieu, Cameron, or Jeff Davis Parish
• Have a child under 21 who is receiving treatment for cancer
• Be employed and covered by health insurance

Required Documents

• Copy of the family's most recent federal income tax form 1040 with signature
• Copy of health insurance cards with benefits information
• Proof of current employment
• Written diagnosis and prognosis from the child's physician
• Estimated out-of-pocket expenses

Application Process

1. Click the "Apply for a Grant" button on the website
2. Enter your name and email address on the profile page
3. Submit to receive an email with a link to the application
4. Use the link to access and complete the application (you can save progress and return at any time)
5. If you do not receive the email within 30 minutes, check your spam or junk folders

Required Materials:

• Copy of the family's most recent federal income tax form 1040 with signature

• Written diagnosis and prognosis from the child's physician
• Estimated out-of-pocket expenses

Note: The application portal allows you to save your progress and return to the form at any time. One grant per family per year is awarded, with a maximum of $5,000 per family.
