Island Health & Wellness Foundation General Grant Program

The Island Health & Wellness Foundation funds health and wellness initiatives from Island-based nonprofits and community organizations.

Geographic Scope: Deer Isle-Stonington community and surrounding areas in Maine

Focus Areas:

• Physical wellness initiatives
• Mental health and wellness
• Substance use prevention, treatment, and recovery
• Medical and dental care services
• School-based health programming
• Senior wellness and healthy aging
• Youth health and wellness programs
• Community health services

Mission: The foundation exists to fund new health initiatives that keep them relevant and responsive to current health and wellness needs of the community.

Key Expectations:

• Programs should demonstrate ability to become self-supporting in a reasonable timeframe
• Organizations must notify the Executive Director within 10 days of any changes to proposed budget or plan
• Fiscal responsibility and sustainability are prioritized
• Some programs that cannot become self-supporting will still be considered for continued funding

• Deer Isle-Stonington CSD 13: School-based health programming and substance use prevention
• Memorial Ambulance Corps: Community paramedicine program
• Salt Air Seniors: Senior wellness and healthy aging activities
• Camp Kooky: Youth camp scholarships for health and wellness programming

Program Description:

1. Detailed project description
2. How the project will positively impact physical wellness of those served
3. Who will benefit from the program/event
4. Number of people you plan to serve (with specific examples)
5. Partner organizations or groups involved
6. Proposed role of IHWF Board of Directors

Budget Information:

1. Total cost of the entire program or event
2. Detailed and complete budget showing IHWF Board costs clearly
3. Amount you are requesting from IHWF
4. Cost of program/event funded by others (specify amounts and sources)
5. List of other funding sources with amounts

Program Assessment:

1. How you will measure/gauge success of the program or event
2. How and when you will disseminate results to IHWF
3. List any similar programs/events currently being offered
4. Whether this is a new program (yes/no)
5. Whether this is a recurring program (yes/no)
6. Any additional information for Board consideration
