Required Application Information

Project Details:
1. Project name
2. Start date and end date
3. Sponsoring organization name
4. Contact person (name, address, phone, email)
5. IH&WF Board member contact (if applicable)

Program Description:
1. Detailed project description
2. How the project will positively impact physical wellness of those served
3. Who will benefit from the program/event
4. Number of people you plan to serve (with specific examples)
5. Partner organizations or groups involved
6. Proposed role of IHWF Board of Directors

Budget Information:
1. Total cost of the entire program or event
2. Detailed and complete budget showing IHWF Board costs clearly
3. Amount you are requesting from IHWF
4. Cost of program/event funded by others (specify amounts and sources)
5. List of other funding sources with amounts

Program Assessment:
1. How you will measure/gauge success of the program or event
2. How and when you will disseminate results to IHWF
3. List any similar programs/events currently being offered
4. Whether this is a new program (yes/no)
5. Whether this is a recurring program (yes/no)
6. Any additional information for Board consideration
