IPHCA Diabetes and Oral Health Integration Project
Funder: Illinois Primary Health Care Association (IPHCA) with funding from DentaQuest Partnership for Oral Health Advancement (DentaQuest Partnership; DQP)
Purpose: To strengthen the capacity of Federally Qualified Health Centers (FQHCs), design and test new models of care, and evolve the safety net to best serve uninsured and underserved Americans. Specifically focuses on medical/dental integration with emphasis on patients with diabetes.
