Homeowner Damage Assistance Program

Status: Support programs are closed at this time (as of April 3, 2026)

Background: Established following the December 10, 2021 tornado that devastated Mayfield and Graves County, Kentucky. This program provides financial assistance to homeowners whose primary residences were damaged by the tornado.

Geographic Scope: Mayfield or Graves County, Kentucky

Eligible Applicants:

  • Residents of Mayfield or Graves County

  • Owned and occupied the damaged property as primary residence on December 10, 2021

  • Household income does not exceed three (3) times the Federal Poverty Level Guidelines for household size at time of loss

  • Only the owner shown on deed/title documents as of December 10, 2021 may apply
  • Award Amount: Lesser of:

  • Value of property damaged (if total loss), reduced by insurance/FEMA/other assistance received

  • Cost of repair (if repairable), reduced by insurance/FEMA/other assistance received

  • $7,500
  • What IS Covered:

  • Damage to residence and contents

  • Mobile homes and modular homes
  • What IS NOT Covered:

  • Detached structures

  • Landscaping, trees, fencing

  • Campers and temporary dwellings
  • Important Notes:

  • Limited funds allocated; not all qualifying applications may receive payment

  • Assistance determined case-by-case

  • Payment made only to owner shown on deed/title documents

  • Application available in English and Spanish

  • Required Information in Application:
    1. Full name (owner on deed/title as of December 10, 2021)
    2. Address of principal residence damaged on December 10, 2021
    3. Previous address if different on December 10, 2021
    4. Phone number
    5. Confirmation property was principal residence on December 10, 2021
    6. Confirmation property was damaged by tornado of December 10, 2021
    7. Current habitability status of property
    8. Description of damage and dollar amount estimate (separate amounts for structure and contents)
    9. Information about renter's/homeowner's insurance (coverage, agent name/contact, deductible amount)
    10. Whether applied to FEMA or other organizations for assistance
    11. Any assistance already received (source and amount)
    12. Number and names of household members residing in home on December 10, 2021
    13. Annual family income (must not exceed three times Federal Poverty Level)
    14. Two references (not household members) aware of the loss

    Required Supporting Documentation:
    1. Copy of deed/title showing applicant as owner (max 256 MB)
    2. Proof of damage: estimate or repair bill if item is not totaled. May use general terms ("all bedroom furniture", "all clothing", "all contents") rather than itemized list unless specifically requested (max 256 MB)
    3. Note from insurance company showing payment made and deductible amount (max 256 MB)
    4. Photograph of damage, if available (max 256 MB)
    5. Proof of income: 2020 or 2021 tax return, wage statement, or other acceptable proof (max 256 MB)
    6. Any additional documentation as circumstances require:
    - 2020 tax return or other proof of income
    - Copy of deed (and title for mobile/modular homes)
    - Letter from insurance agent with deductible information
    - Proof of property value
    - Proof of FEMA denial
