The Research Institute for Home Care Research Grant is the only grant dedicated solely to the betterment and understanding of care at home. Topics may include:

• Clinical innovation
• Hospital-to-home transitions
• Care for culturally and racially diverse patients
• Home Health Value Based Purchasing (HHVBP)
• Medicare Advantage in home health

• Deprescribing and polypharmacy management

Eligibility:

• Researchers and research teams from academic institutions, health organizations, and healthcare systems
• Both academic and community-based researchers eligible
• Multi-disciplinary teams encouraged

Objective: To advance home care research, improve healthcare delivery at home through quality and innovation, inform policy, and identify best practices and patient care models.

Mission: The Institute invests in research and education about home health and hospice care and its ability to deliver quality, cost-effective, patient-centered care across the care continuum.
