Summary

The HFCM Activation Fund Grant supports one-year projects aimed at enhancing community organizations' capacity and effectiveness. Eligible projects may include innovative health strategies, partnerships addressing key health issues, exploration of emerging health challenges, and improvements to service delivery facilities. Organizations are encouraged to consult with The Health Foundation at least 30 days prior to the LOI deadline to align their proposals with the Fund's goals.

Overview

NOTE: Organizations interested in applying are strongly encouraged to attend an online information session and contact the Senior Program Officer to discuss alignment of proposed projects with Activation Fund goals prior to submitting a letter of intent. We encourage applicants to consult with The Health Foundation staff at least 30 days before the LOI deadline. Activation Fund The Activation Fund provides support for discrete, one-year projects designed to help community organizations move to the next level of capacity and effectiveness. This may involve one or more of the following: Developing creative and innovative approaches to alleviate community health concerns; Enabling partnerships with other organizations to address health issues with new strategies; Exploring emerging health challenges and opportunities; and/or Improving facilities to enhance or expand service delivery.
