Background: Housing can be a powerful platform for health. For this to be true, both affordability and opportunity must be present. The Foundation released this RFP in August 2018 under the Housing Wrap-Around Services focus area.

Program Goals:

• Increase opportunity available to residents of affordable housing communities
• Create conditions that foster self-sufficiency and/or stabilized health for low-income individuals and families

Project Duration: Two years

Target Population: Residents of affordable housing communities; low-income individuals and families

Eligibility: Organizations providing housing or wrap-around services
