Health - Integration, Collaboration & Advocacy The California Community Foundation is seeking letters of interest for grants supporting work in improving immigrant health access, behavioral health and health policy and advocacy. All potential applicants are strongly encouraged to speak with a Program Officer before beginning the application process to learn more about the current emphases. Strategies & Outcomes To promote and improve the health and quality of life of children and adults living in Los Angeles County, the foundation focuses on grants to organizations that address the following topics: Increasing integration of medical, dental, vision, mental health and substance use prevention, care and treatment servicesFacilitating cross-sector and regional systems collaboration among L.A. County public sector partners, nonprofit hospital/health systems, other nonprofits, and community partners to address the root causes of racial/ethnic health disparities including but not limited to immigration statusSupporting health policy/advocacy efforts that promote universal access to and utilization of health coverage, services and other public benefits

Eligible organizations are:Nonprofit agencies with evidence of tax-exempt status under Section 501 (c)(3) of the Internal Revenue Code and not classified as a private foundationLocated within and primarily serving residents of Los Angeles County, except for regional, statewide or national public policy efforts that may benefit a substantial portion of the local populationOperated and organized so that they do not discriminate in the hiring of staff or in providing services on the basis of race, religion, gender, sexual orientation, age, national origin or disability
