GRACE Fund - Giving Relief And Care for Employees

Overview: A crisis-based emergency financial assistance program established to help Self Regional Healthcare team members who suffer a catastrophic situation.

Purpose: Provide emergency financial assistance to employees and their families during major, life-altering crises

Geographic Scope: Self Regional Healthcare employees and their families

Eligibility:

Self Regional Healthcare team members (employees)

Spouse and legal dependents up to age 18, or 25 if full-time college student

Crisis Criteria (ALL must be met):

- The crisis must be a one-time occurrence
- The crisis must be of a devastating, major, serious, life-altering nature
- Team member must have exhausted other available resources that would impact the crisis

Award Details:

Maximum Award: Up to $1,000 per application

Review Process: Applications are reviewed by a full committee

Funding: The GRACE Fund is funded by team member contributions to the Foundation's Team Member Campaigns
