Award Access Options

Once awarded, recipients receive a TERMS OF AWARD document with three options for accessing funds:

Option One - Reimbursement: For items/services already purchased. Submit proof of payment such as:

  • Copy of check

  • Official paid receipt from vendor

  • Register receipt with place, date, item, and cost
  • Option Two - Direct Payment to Vendor: For applicants lacking upfront funds. Provide:

  • Invoice or bill for payment

  • Estimate from vendor if bill unavailable prior to purchase
  • Option Three - Gift Card Purchase: For items from major vendors (Walmart, Amazon, etc.). Provide:

  • Vendor/retailer name

  • Item cost
  • Application Support

    Applicants without computer access are encouraged to seek assistance from case managers, care managers, physician's offices, vendors, or other organizations involved in their care.
