  • Background: The Clarksville-Montgomery County Community Health Foundation is a public advocacy board entrusted with disbursing public assets flowing from Clarksville Volunteer Health Inc., the 20% minority joint-venture partner in Gateway Medical Center. Revenues and receipts from publicly owned assets in Gateway are transferred to the Community Health Foundation for distribution to various health-related initiatives.

  • How to Apply

    Application Process

    Step 1: Contact the Grants Manager to schedule a meeting to discuss your proposal. This meeting should occur at least 30 days prior to the grant deadline.

    Step 2: If your proposal is determined to be eligible for funding, you will receive:

  • CMC Community Health Foundation Grant Application

  • Outputs and Outcomes Expectations Report
  • Step 3: Submit your completed application by email by 5pm on the application deadline date.

    Application Deadlines

    There are two application windows per year:

  • Application Window #1: January 1st - May 1st

  • Application Window #2: July 1st - November 1st
  • Required Materials

    • Grant Application Form
    • Outputs and Outcomes Expectations Report (with specific goals and objectives)
    • Documentation of organizational eligibility
    • Budget documentation
    • Project timeline and deliverables

    Important Notes

    • All applicants must contact the Grants Manager prior to submitting an application
    • Applicants who submit early may request feedback from the Grants Manager prior to final submission
    • Applications must be submitted by email
    • Submit early for maximum support from the Grants Manager

    Follow-Up Requirements

    Final Report: Grant recipients must submit a final report detailing progress in accomplishing grant goals and objectives.

  • Due: ONE month after the contract ends

  • Failure to submit by the due date will result in ineligibility for funding in subsequent cycles

  • Photos of the project are welcomed

  • Photo release form required with report
  • Grant Modifications

    If a grant requires modification during the contract period:

  • Grantee must contact the Grants Manager for a modification form

  • Grants cannot be modified without approval and completion of the Grant Modification Form
