7. Program description (must include purpose, audience, and quantitative objectives)
8. Is this a one-time initiative or ongoing? If ongoing, have you identified future funding sources?
9. Project timeline including start and completion dates and/or assessment dates
10. If you receive a portion of grant request, will you proceed with the program?
11. Detailed budget (file upload, max 20MB) that includes:
- Financial contributions made by credit union and other partners
- Whether expenses are estimates or actual costs
- Specification of what budget items Minnesota Credit Union Foundation grant will cover
12. Are other organizations involved in the project? Are they 501(c)(3)? Describe their involvement.
13. Explanation of how grant will benefit credit union and/or community
