Excellence in Care Awards

Funder: DIFFA/Chicago (Design Industries Foundation Fighting AIDS/Chicago)

Geographic Scope: Chicagoland area (Chicago and surrounding suburbs)

Mission Alignment: DIFFA Chicago's grant-making tradition supports programs and activities that reflect a unique or innovative approach to addressing the impact of HIV/AIDS in the Chicagoland area and whose programs align with Get To Zero Illinois.

    Focus Areas

    Grants support agencies and programs across Chicago and surrounding suburbs that focus 100% of their efforts on HIV/AIDS, with particular emphasis on:
  • At-Risk Youth (ages 12-21)
  • Men who have Sex with Men (MSM), specifically Men of Color
  • LGBTQ programs specifically serving the South and West sides of Chicago

    Eligible Organizations

  • Community-based 501(c)(3) organizations
  • Organizations providing direct service, preventive education, and outreach to people who are:
  • - HIV positive - Living with AIDS - At-risk of infection

    What DIFFA Chicago Seeks

  • Programs reflecting unique or innovative approaches to HIV/AIDS impact
  • Organizations where public support and other philanthropies are not otherwise available
  • Grant funds positioned to expand and diversify service program impact
  • Proposals that include appropriate and strategic collaboration among agencies
  • Chicago agencies providing the best care with dollars having the most impact

How to Apply

Grant Application Process

Note: DIFFA Chicago does not accept unsolicited grant proposals. Invitations are sent to eligible organizations to apply.

#### Step 1: Application Invitations

  • Invitations for applications from community organizations go out in Fall

  • Application includes:

  • - How the money would be used
    - Summary description of the program
    - Financial documents

    #### Step 2: Board Review

  • Initial review of applications by DIFFA Chicago Board
  • #### Step 3: Grant Review Teams

  • Establishment of Grant Review Teams from board members

  • Each team assigned a set number of applications for in-depth review

  • One-on-one interviews with applicant organizations scheduled
  • #### Step 4: In-Person Interviews

  • In-person, on-site interviews held between Grant Review Teams and Applicant Organizations

  • Virtual interviews used when health concerns dictate

  • Review of:

  • - Facilities
    - Program details
    - Provided documentation

    #### Step 5: Organizational Ranking

  • Based on interviews and documentation review

  • Rankings established by each Grant Review Team

  • Against consistent and universal criteria
  • #### Step 6: Executive Committee Review

  • Rankings provided to DIFFA Chicago Executive Committee

  • Recommendations made to the Board
  • #### Step 7: Board Presentation and Vote

  • Grant Review Teams present findings to full Board

  • Board debates and establishes recommendations for grant awards and dollar amounts

  • Board votes on final decisions
  • #### Step 8: Official Announcement

  • Official announcement of Grantee Organizations made at Excellence in Care Awards in December

  • Awards supported through social media visibility campaign
